Every permanent residence application to Canada needs an immigration medical exam, and most students, workers and Super Visa applicants from Pakistan need one too. It is a short appointment, but a missing document or a booking made at the wrong time can hold up your whole application.

Who Needs a Canada Immigration Medical Exam?

All permanent residence applicants must complete an immigration medical exam, and so must their spouse or partner and dependent children, even if they are not moving to Canada.

Pakistani applicants also need an exam in these cases:

  • Study, work and visitor stays over 6 months: Required if you lived in Pakistan, or another designated country, for at least 6 consecutive months during the year before entering Canada. See our Canada study visa guide.
  • Super Visa: Required for every parent and grandparent applying. Read our Canada Super Visa guide.
  • Public health jobs: Required for work in health care, clinical labs, long-term care, primary or secondary schools, child care, domestic work or in-home caregiving, regardless of how long you stay or where you lived.

Visitors staying 6 months or less usually do not need an exam.

Exemption for applicants already in Canada: Applicants living in Canada who are applying for PR or temporary residence may be exempt until 5 October 2029 if an exam in the last 5 years showed low or no risk to public health or safety.

  • Super Visa: Complete the exam before applying, since proof of the exam must be included.
  • Other PR applications: Wait for IRCC’s medical request after you apply, then complete the exam within 30 days.
  • Study and work permits: Take an upfront exam before applying, or wait for IRCC’s request.

Results are valid for 12 months, so schedule an upfront exam close to your application date to avoid needing a second exam.

Documents to Bring to Your Medical Exam

  • Health request letter: IMM 1017E or IMM 1020E with your UCI or IME number (not needed for an upfront exam)
  • Medical records: Reports and test results for past or current conditions, and a list of medicines you take
  • Glasses or lenses: If you wear them
  • Polio certificate: Your valid polio vaccination certificate, which you also need to travel out of Pakistan
  • Antenatal card: If you are pregnant
  • Children under 16: A parent or legal guardian with proof of relationship, such as the NADRA FRC or the mother’s marriage certificate naming the father

Photos are not needed, as IOM uses IRCC’s eMedical system and takes them at the centre.

What to Expect During a Canada Immigration Medical Exam

The exam takes about 3 to 4 hours and covers your medical history, a physical examination and tests based on your age. No fasting is needed, you can request a chaperone at any stage, and any vaccinations offered at the centre are voluntary.

Exam Day Steps

  1. Registration: IOM staff check your identification and health request letter, then take your photo.
  2. Medical history: You complete a questionnaire about past illnesses, surgeries and current medicines. Disclose every condition, as IOM reports concealment to IRCC.
  3. Physical examination: A panel physician checks your weight, height, hearing, vision, heart, lungs, abdomen, limbs and skin. You change into a gown but keep your underwear on, and the exam does not include genital or rectal checks.
  4. Tests: Depending on your age, you give a urine sample, have a chest X-ray and give a blood sample.
  5. Follow-up, if needed: The physician may ask for extra tests, such as sputum samples, or a specialist consultation.

Canada Immigration Medical Exam Results and Validity

IOM sends your results directly to IRCC through eMedical once all reports are complete. You receive an information printout sheet showing your IME number, not the results themselves. IRCC writes to you only if it needs more information, tests or treatment.

Upfront applicants, including Express Entry candidates, upload this sheet with their application.

Results are valid for 12 months and must still be valid when you apply, according to IRCC’s medical exam requirements. Your PR visa or confirmation of permanent residence (COPR) usually expires on your medical expiry date, so you must land in Canada before then or take a new exam.

Tuberculosis (TB) Follow-Up and Medical Surveillance

  • Sputum tests: If the chest X-ray suggests active TB, you give sputum samples on three consecutive mornings, and lab results take 8 to 10 weeks.
  • Active TB: Treatment lasts at least 6 months and must be completed before IRCC continues with your application.
  • Inactive TB: You must contact provincial public health authorities within 30 days of arriving in Canada for medical surveillance.

Common Mistakes That Delay a Canada Immigration Medical Exam

  • Invalid identification: An expired passport or an unclear passport copy can lead to a rescheduled appointment.
  • Missing health request letter: Applicants without an upfront exam need the IMM 1017E or IMM 1020E showing their UCI or IME number.
  • Undisclosed medical history: IOM reports deliberate concealment to IRCC, and misrepresentation can lead to refusal and a 5-year ban from applying to Canada.
  • Booking only the main applicant: Every family member needs their own exam, including those not moving to Canada.
  • Missing guardian proof: Children under 16 cannot be examined without a parent or guardian and proof of relationship.
  • Illness on the day: Reschedule if you or your child has a fever or rash.
  • Delayed follow-up: Missing IOM’s deadline for sputum samples or extra tests postpones the submission of your results to IRCC.
